This report provides aggregate information about participant referrals to other sites/programs and entities. A hyperlinked report provides detailed information about participants based on reason for referral.

Prompts:

Site From: required prompt to select the Site or Sites that should be included in the report. Select at least one site to decrease run time of report. Only sites that referrals are made from will be included; this does not limit based on which site or sites a participant is currently active in.

Referral Date Start: optional prompt to select the beginning of the date range.  Only referrals made after or on this date will be included.  Select a start date to decrease run time of the report.

Referral Date End: optional prompt to select the end of the date range.  Only referrals made before or on this date will be included.  Select an end date to decrease run time of the report.

Program From: optional prompt to select Program or Programs that should be included in the report. Select at least one program to decrease run time of report. Only programs that referrals are made from will be included; this does not limit based on which program or programs a participant is currently active in.

Sorts:

This report is sectioned first by Site, then Program, then Point of Service.  Each section is displayed in alphabetical order.

Notes:

For each site the following summary information is listed:

-          Number of Referrals in the Date Range – an unduplicated count of referrals made in the site

-          Number of Unduplicated Participants Referred – an unduplicated count of participants that have referrals made during the date range in the site

For each program the following summary information is listed:

-          Number of Referrals in the Date Range – an unduplicated count of referrals made in the program

-          Number of Unduplicated Participants Referred – an unduplicated count of participants that have referrals made during the date range in the program

For each program, there are tables with the following information:

-          Type of Referral – the type selected when the referral was made; does not distinguish between types of entities that were referred to

-          Number of Referrals – a count of referrals made that corresponds to the listed referral types; the count serves as a hyperlink that launches a detailed list of the participants that were referred in the program for the selected type.

-          Percent –  the percent of total referrals made that corresponds to the listed referral types

-          Reason for Referral – the reason selected when the referral was made

-          Number of Referrals – a count of referrals made that corresponds to the listed reasons for referral

-          Percent – the perfect of total referrals made that corresponds to the listed reasons for referral

For each program the following pie charts are displayed:

-          Type of Referrals – this pie chart displays visually the data in the corresponding table

  •          Reason for Referral – this pie chart displays visually the data in the corresponding table
