Creates a field with a drop-down menu of Entities. The administrator chooses the type of Entity that will populate the drop-down menu. Only one type of Entity can be selected; a corresponding Entity Sub-Type may also be selected, but the Sub-Type is not required.

Only Entities currently active and enrolled in the Program that the form is being completed in will appear in the drop-down menu.
